How To Apply

    Keeping in mind our Franciscan values, please describe why you want to attend USF, the reasons for choosing your major, what you know about the profession, and how you plan to be successful at USF.

  6. Academic Skills Testing
    All incoming undergraduate students must complete the Test of Essential Academic Skills (TEAS). The test is offered monthly at USF Crown Point per the schedule below:

    • First Tuesday of each month at 5:30 p.m.
    • Third Friday of each month at 9:00 a.m.

Program-Specific Information & Requirements

If you are applying the following programs, you will have additional requirements.

Applied Science

In order to be admitted to our Applied Science degree program, incoming students must be a graduate of an accredited Emergency Medical Services (EMS) program.

Certified Nurses Aide

To be considered for admission to the CNA course at USF Crown Point, you must:

  • Complete a criminal background check (prior to starting clinical experience)
  • Complete a drug screen (prior to starting clinical experience)
  • Receive a physical exam
  • Obtain up-to-date titers and necessary vaccines
  • Have a GPA of 2.0 or higher, or have a GED of 600 with no category less then
  • 150 after 2014; prior to 2014, a subscore of 500 is required
  • Complete the clinical packet requirements
  • Pay course tuition of $800 in full by noon the Friday prior to the start date

In addition, you will need to obtain:

  • Textbook
  • Stethoscope
  • Uniform (white scrub top and pants, white nurse shoes)
  • Watch with second hand

Program Requirements

In the CNA course, you’ll receive an introduction to the health care delivery system and learn the specific requirements of the CNA in accordance with the Indiana State Department of Health.

Through classroom coursework, skills laboratories, hands-on experience in USF Crown Point’s simulation laboratory, and supervised clinical experiences in a long-term care facility, you will obtain a complete understanding of your role as a CNA.

In addition, you will gain basic knowledge and skills related to:

  • The aging process
  • Verbal and nonverbal communication
  • Residents’ rights
  • Patient care procedures
  • Hygiene
  • Mobility
  • Restorative services

Licensed Practical Nurse to RN (LPN-RN)

In addition to meeting all USF entry requirements, candidates must:

  • Submit a current active LPN license
  • Have a GPA of 2.7 or higher from the LPN program attended
    • If you graduated from a non-accredited LPN program and meet all other minimum admission requirements, you will be eligible for admission as a Pre-Nursing student.
  • Have completed the TEAS

Medical Laboratory Technician, Associate of Science

All students are admitted to the University as Pre-MLT majors. During year one, students complete general education and basic science coursework. In year two (the clinical year), up to 12 students transition into the MLT program to complete clinical classes, labs, and clinical rotations.Requirements for the clinical year include:

  • Minimum college cumulative GPA of 2.5 on a 4.0 scale in all college work attempted
  • Minimum college science GPA of 2.5 on a 4.0 scale
  • Grade of “C” (or greater) in all general education and basic science courses
  • Completion of all courses general education and basic science courses or equivalent before the start of the fall semester of the clinical year
  • Ability to meet the non-academic Essential Functions of the program

M.S. in Clinical Mental Health Counseling

Admissions Requirements

Candidates for must have the following

  • Bachelor’s degree from a regionally accredited institution in the U.S.
  • An undergraduate GPA of at least 3.0 on a 4.0 scale
  • Completed nine and 12 hours of undergraduate coursework in psychology (may be augmented by coursework in closely related fields and/or reflect life experience)
  • A commitment to counseling profession
  • Quality written and oral communication skills

To Apply

In addition to the USF Crown Point Application, candidates must submit the following:

  • Official transcripts from all postsecondary institutions attended; these must be sent directly from the registrar of institution(s) attended. Use our Transcript Request Form.
  • A concise, typed, two-page statement of your professional goals, including why you wish to become a licensed counselor. You could also include specific training objectives and long-term career goals following graduation.
  • Three letters of recommendation (at least one professional and one academic reference); Must be submitted using our department form.

After the Office of Admissions has received the above-listed materials, you will be required to interview with the Psychology and Counseling Admissions Committee.

Phlebotomy Course

To be considered for admission to the Phlebotomy course at USF Crown Point, you must:

  • Complete a criminal background check (prior to starting clinical experience)
  • Complete a drug screen (prior to starting clinical experience)
  • Have a current CPR card
  • Be at least 18 years old
  • Have a High School GPA of 2.0 or higher, or have a GED of 600 with no category less than 150 after 2014; prior to 2014, a subscore of 500 is required
  • Pay course tuition of $1,735.00 in full by noon the Friday prior to the start date which includes National Certification Exam Fee.

RN-BSN

RN-BSN applicants (Registered Nurse to Bachelor of Science in Nursing) must also submit

  • Proof of a current, active RN license.
  • An official transcript from an associate or diploma degree nursing program.
    • A minimum cumulative GPA of 2.7 on a 4.0 scale is required.
